AbstractAlthough the 2008 report of the WHO Commission on Social Determinants of Health has influenced the development of a number of recent national health policies (e.g. Norway, Spain), the public health literature still provides few insights on how health promotion systems can address the social determinants of health (SDOH). In this presentation, we describe how we intent to facilitate and study the transition from programs largely centred on health-related risk factors to programs integrating the SDOH to reduce inequalities in health. More specifically we present 1) the conceptual framework guiding our interventional research, 2) the different components of our strategy, and 3) the alliances we forged with key regional and local partners.

We use Solar and colleagues’ (2007) conceptual Framework for Action on the SDOH to map out the array of actors defining the health promotion system of Pays de Redon-Bretagne Sud; an impoverished sub-region of Brittany and Pays de Loire. The project was first initiated through on-going discussions between the representatives of the Mutuelle des Pays de Vilaine (MPV: a not-for-profit health insurance company) and researchers from the EHESP (French School of Public Health). The target of the intervention program is the MPV and its partners sitting on a committee especially set up to develop a joint health promotion program. The committee includes representatives of the health, social and economic sectors, local and regional governments and elected officials.

The EHESP team, in collaboration with a long-established NGO in health promotion (IREPS Bretagne), will provide the committee members and their staff with training, tools for program planning, implementation and evaluation and also with analyses of the existing actions and targets. This intervention research will provide an in-depth account of the challenges and barriers paving the way to better integration of the social determinants of health by health promotion systems.
